• Ongoing coverage for maintenance plans
• Proper access to treatment areas
• Following prescribed prevention guidelines

4. Payment Terms

• At time of service for one-time treatments
• Monthly/quarterly for ongoing services
• Before service for emergency calls

5. Cancellation Policy

Please provide:

• 24-hour notice for regular service cancellation
